Goal: Determine whether adding intraperitoneal IMNN-001 to standard neoadjuvant and adjuvant chemotherapy improves overall survival in newly diagnosed advanced ovarian, fallopian tube, or primary peritoneal cancer, while assessing safety.

Patients: Women aged 18 years or older with previously untreated, high-grade non-mucinous epithelial ovarian, fallopian tube, or primary peritoneal cancer at FIGO stage IIIB/C or IV who are candidates for neoadjuvant chemotherapy. Patients need ECOG performance status 0–2, adequate organ function, and tumor tissue for homologous recombination deficiency testing. Key exclusions include bowel obstruction or other conditions preventing intraperitoneal catheter placement, active autoimmune disease requiring treatment, and planned bevacizumab.

Design: Adaptive, open-label, multicenter trial with 1:1 randomization, stratified by tumor homologous recombination status and stage. Both groups undergo interval debulking surgery after approximately three neoadjuvant cycles, followed by three adjuvant cycles and standard-of-care maintenance therapy. An independent committee monitors safety and study conduct.

Treatments: Both groups receive paclitaxel 175 mg/m² and carboplatin AUC 6 intravenously every three weeks for six cycles, followed by standard-of-care maintenance therapy, which may include olaparib or niraparib. The experimental group also receives IMNN-001 100 mg/m² intraperitoneally for a planned 17 doses across the neoadjuvant and adjuvant periods. IMNN-001 is an investigational lipopolymer-delivered plasmid encoding interleukin-12, intended to stimulate a local antitumor immune response in the peritoneal cavity. In the prior randomized OVATION-2 study, median progression-free survival was 14.9 versus 11.9 months and median overall survival was 46.0 versus 33.0 months with IMNN-001 plus chemotherapy versus chemotherapy alone; these numerical differences did not establish a definitive survival benefit.

Outcomes: The primary endpoint is overall survival from randomization through 48 months. Secondary measures include pathologic chemotherapy response score and residual disease status at interval debulking surgery, objective response before surgery by RECIST v1.1, and time to second-line treatment or death.

Burden on patient: The experimental arm has a high additional treatment burden relative to standard chemotherapy: intraperitoneal catheter placement and 17 intraperitoneal administrations require frequent visits, including visits between chemotherapy cycles. Both groups undergo tumor sampling for biomarker testing, chemotherapy, interval debulking surgery, and follow-up assessments; no intensive pharmacokinetic sampling is specified.
