Goal: Identify a suitable dose of HLX43 and evaluate its antitumor activity, safety, and tolerability in previously treated advanced NSCLC.

Patients: Adults with unresectable stage IIIB/IIIC or metastatic NSCLC, measurable disease, ECOG performance status 0–1, and adequate organ function. Patients without actionable genomic alterations must have received PD-(L)1 therapy and platinum chemotherapy; those with actionable alterations must have received appropriate targeted therapy and platinum chemotherapy. Key exclusions include prior topoisomerase I–targeting treatment, active CNS metastases, clinically significant pulmonary disease, and prior grade ≥3 immune-related adverse events.

Design: Global, multicenter, open-label phase II study randomizing eligible patients 1:1 between two HLX43 dose levels; there is no standard-treatment control arm.

Treatments: Both groups receive intravenous HLX43 every three weeks until progression, unacceptable toxicity, initiation of another anticancer therapy, death, or consent withdrawal. HLX43 is an investigational PD-L1–directed antibody–drug conjugate carrying a topoisomerase I inhibitor; it may combine PD-L1 pathway blockade with delivery of a DNA-damaging payload, although the clinical contribution of each mechanism is uncertain. Preliminary phase I/II NSCLC subgroup analyses reported investigator-assessed response rates of 36.4%–50.0% at reported doses, without a standard-treatment comparison; grade ≥3 treatment-related adverse events occurred in 42.9% of 205 treated patients.

Outcomes: The primary endpoint is objective response rate by independent radiology review using RECIST 1.1. Secondary endpoints include investigator-assessed response rate, progression-free survival by independent review and investigator assessment, overall survival, and adverse-event incidence and severity, with assessment up to 24 months.

Burden on patient: Moderate to high: patients must travel for intravenous infusions every three weeks and undergo repeated imaging, laboratory testing, and safety assessments. Tumor tissue is required for PD-L1 testing, potentially necessitating a new biopsy if suitable archived tissue is unavailable. Ongoing treatment and follow-up may extend until progression or intolerance; intensive pharmacokinetic sampling is not specified.
