Goal: Evaluate the antitumor activity and safety of ivonescimab in advanced, incurable salivary gland carcinoma.

Patients: Adults with recurrent, metastatic, or unresectable salivary gland carcinoma, including adenoid cystic carcinoma, who have ECOG performance status 0–1, measurable extracranial disease, and recent evidence of progression or worsening symptoms. Prior systemic therapy is not required, but prior oral VEGFR tyrosine kinase inhibitors or anti–PD-1 therapy are not permitted.

Design: Open-label, nonrandomized, single-agent study with separate adenoid cystic and non–adenoid cystic carcinoma cohorts. An interim response assessment after approximately 22 patients across the cohorts determines whether enrollment continues.

Treatments: Ivonescimab is administered on day 1 of each 21-day cycle. It is an investigational bispecific antibody targeting PD-1 and VEGF-A, designed to address immune suppression and tumor angiogenesis. Phase 3 studies in advanced non–small-cell lung cancer have shown improved progression-free survival in several settings, but those results do not establish efficacy in salivary gland carcinoma.

Outcomes: The primary endpoint is RECIST v1.1 objective response rate. Secondary endpoints include progression-free survival, overall survival, duration of response overall and by cohort, and adverse events graded by CTCAE v5.0.

Burden on patient: Moderate to high: treatment requires clinic visits every 3 weeks, imaging every 9 weeks during treatment, an end-of-treatment assessment, and follow-up every 12 weeks. Screening and monitoring include blood and urine tests and potentially a tumor biopsy if safe and feasible; participation, including follow-up, is expected to last approximately 3 years.
