Goal: Determine a safe recommended dose of A2B395 and assess its antitumor activity and safety in EGFR-expressing solid tumors with loss of HLA-A*02 expression.

Patients: Adults with previously treated, recurrent unresectable, locally advanced, or metastatic colorectal cancer, NSCLC, head and neck squamous cell carcinoma, triple-negative breast cancer, renal cell carcinoma, or another EGFR-expressing solid tumor. Patients must be germline HLA-A*02 heterozygous, have tumor HLA-A*02 loss confirmed through BASECAMP-1, measurable disease, and ECOG performance status 0–1.

Design: Multicenter, open-label, single-arm study with phase 1 dose finding followed by phase 2 evaluation at the recommended dose. There is no randomized comparator.

Treatments: Patients receive preconditioning lymphodepletion followed by one intravenous infusion of A2B395. This donor-derived, logic-gated CAR T-cell product uses an EGFR-targeting activator to recognize tumor cells and an HLA-A*02-targeting blocker intended to spare healthy cells that retain HLA-A*02. In early company-reported results, 2 of 9 response-evaluable patients had a complete or partial response; among 10 treated patients, two had grade 1–2 cytokine release syndrome, with no reported dose-limiting toxicities. These small, nonrandomized findings do not establish efficacy or long-term safety.

Outcomes: Phase 1 measures adverse events and dose-limiting toxicities and selects the recommended phase 2 dose. The phase 2 primary endpoint is independently reviewed objective response rate by RECIST 1.1. Secondary measures include CAR T-cell persistence and blood cytokine levels, with follow-up extending to 24 months after infusion.

Burden on patient: High. Participation requires separate BASECAMP-1 prescreening and tumor molecular testing, lymphodepleting chemotherapy, an intravenous cell infusion, and close safety monitoring for potentially serious CAR T-cell toxicities. Serial blood sampling, imaging and assessments through 24 months, plus long-term safety follow-up, add visits and travel; the exact visit and sampling frequency is not specified.
