LNS8801 is an investigational oral drug being studied for advanced cancers, particularly melanoma, both alone and with pembrolizumab. Human results have been reported, but they come from small, early-phase groups; a randomized study in previously treated cutaneous melanoma is underway. (ascopubs.org)
LNS8801 activates the G protein-coupled estrogen receptor (GPER). It is the active enantiomer of the research compound G-1. Laboratory and animal studies found GPER-dependent anticancer effects, including reduced cancer-cell proliferation; whether these findings translate into clinical benefit remains under investigation. (pmc.ncbi.nlm.nih.gov)
These small, nonrandomized groups cannot establish how much benefit is attributable to LNS8801, particularly when it is combined with pembrolizumab. (jitc.bmj.com)
In the 10-patient cutaneous-melanoma combination group, 8 had adverse events considered possibly related to treatment, including 4 with grade 3 events; elevated liver enzymes, diarrhea and fatigue each occurred in more than one patient. In the nine-patient LNS8801-alone group, 7 had possibly related adverse events, including 2 with grade 3 events. The small numbers limit conclusions about uncommon risks. (jitc.bmj.com)

Goal: Determine whether LNS8801 alone or with pembrolizumab improves outcomes versus physician’s choice treatment in checkpoint inhibitor–refractory cutaneous melanoma, while assessing safety and tolerability.
Patients: Patients with unresectable or metastatic cutaneous melanoma, measurable disease, ECOG performance status 0–1, and progression after anti–PD-1 therapy. Enrollment requires two copies of the specified functional GPER sequence (C/C), confirmed by prescreening blood test. Prior anti-CTLA-4 and/or BRAF-directed treatment is required unless the patient is ineligible or has declined it.
Design: Multicenter, open-label, randomized controlled trial with 1:1:1 allocation to three arms. Randomization is stratified by baseline LDH, number of disease sites, and primary versus secondary resistance to anti–PD-1 therapy.
Treatments: Patients receive oral LNS8801 125 mg daily, the same dose plus intravenous pembrolizumab 200 mg every 3 weeks, or physician’s choice of dacarbazine, temozolomide, pembrolizumab, nivolumab/relatlimab, or nivolumab/ipilimumab. Treatment may continue for up to approximately 2 years, with selected patients permitted to continue beyond progression. LNS8801 is an investigational GPER agonist; preclinical work suggests GPER-dependent inhibition of cancer-cell proliferation. In a small, nonrandomized cutaneous melanoma cohort, LNS8801 plus pembrolizumab produced partial responses in 2 of 10 patients, whereas a separate LNS8801 monotherapy cohort reported stable disease in 5 of 8 evaluable patients and no objective responses. These early findings do not establish clinical benefit.
Outcomes: The primary endpoint is RECIST v1.1 progression-free survival, principally comparing LNS8801 plus pembrolizumab with physician’s choice. Secondary measures include progression-free survival comparisons involving monotherapy, overall survival, response rate and duration, disease control, treatment-emergent and serious adverse events, and hospitalization days. Exploratory assessments include quality of life, metabolic measures, and outcomes by prior anti–PD-1 resistance and disease characteristics.
Burden on patient: Moderate. Prescreening requires a genetic blood test, followed by full screening if eligible. Patients take daily tablets or attend infusion visits according to assignment, undergo imaging every 8 weeks in year 1 and every 12 weeks in year 2, and have periodic safety assessments, including blood tests approximately every 6 weeks. These visits and the first-year imaging schedule may exceed routine care, but no mandatory research biopsies or intensive pharmacokinetic sampling are described.
