Goal: Assess whether zanzalintinib monotherapy can control advanced leiomyosarcoma, primarily by measuring progression-free survival at 6 months.

Patients: Adults with metastatic or unresectable, measurable leiomyosarcoma who have received more than two prior lines of anticancer therapy, including no more than two tyrosine kinase inhibitors. Patients need good performance status (ECOG 0–1 or Karnofsky >70%) and adequate organ and cardiac function. Key exclusions include prior zanzalintinib, untreated or unstable brain metastases, and significant cardiovascular, bleeding, or gastrointestinal risks.

Design: Open-label, single-arm phase II study without randomization or a comparator group. Treatment continues until progression or unacceptable toxicity.

Treatments: Patients take oral zanzalintinib (XL092) once daily in repeating 14-day cycles. This investigational multikinase inhibitor targets VEGFR2, MET, and the TAM-family kinases TYRO3, AXL, and MER, which are involved in angiogenesis and tumor growth. In previously treated metastatic colorectal cancer, early-study zanzalintinib monotherapy produced a 1.9% response rate and median progression-free survival of 3.0 months; these results do not establish efficacy in leiomyosarcoma.

Outcomes: The primary endpoint is 6-month progression-free survival. Secondary endpoints are median progression-free survival, overall survival, confirmed objective response rate, duration of response, and adverse events.

Burden on patient: Estimated moderate burden: treatment is taken at home, but patients need screening cardiac imaging, on-study blood draws, and repeated CT scans. Cardiac imaging may be repeated if clinically indicated. After treatment, follow-up occurs at 30 days, every 12 weeks for 2 years, then every 6 months for up to 5 years from treatment start. No mandatory research biopsy or intensive pharmacokinetic sampling is specified.
