Goal: Assess the activity and safety of sequential antibody–drug conjugates with different targets in HER2-negative metastatic breast cancer, particularly whether switching agents after progression retains antitumor activity.

Patients: Adults with measurable, unresectable locally advanced or metastatic HER2-low or HER2-0 breast cancer, either hormone receptor–positive or –negative, and ECOG performance status 0–1. First-treatment cohorts allow 0–1 prior metastatic chemotherapy lines; second-treatment cohorts require progression on the other study drug as the most recent therapy. Patients with hormone receptor–positive disease who are candidates for endocrine therapy must have received a CDK4/6 inhibitor. Selected patients with brain metastases are eligible. Prior HER2-positive disease and clinically significant pneumonitis or interstitial lung disease generally exclude participation.

Design: Multicenter, open-label, randomized, non-comparative phase II study. Participants entering the first-treatment group are assigned to trastuzumab deruxtecan or datopotamab deruxtecan within hormone receptor–defined cohorts. After progression, eligible participants may cross over to the other drug; patients who have already progressed on one drug may enter the second-treatment group directly.

Treatments: Trastuzumab deruxtecan (T-DXd) and datopotamab deruxtecan (Dato-DXd) are given on day 1 of each 21-day cycle until progression or loss of benefit, with eligible patients switching agents after progression. T-DXd is a HER2-directed antibody–drug conjugate that delivers a topoisomerase I inhibitor and is approved for previously treated HER2-low metastatic breast cancer. Dato-DXd targets TROP2 and delivers the same class of cytotoxic payload through a cleavable linker. In the phase III TROPION-Breast01 trial in previously treated hormone receptor–positive, HER2-negative metastatic breast cancer, Dato-DXd improved progression-free survival versus chemotherapy (hazard ratio 0.63; objective response 36.4% versus 22.9%). Those results do not establish its efficacy after T-DXd; activity of either treatment after progression on the other is being investigated here.

Outcomes: The primary endpoint is RECIST 1.1 objective response rate separately for the first- and second-treatment cohorts. Secondary endpoints include progression-free and overall survival, clinical benefit rate, time to progression, time to response, duration of response, grade 3–5 treatment-related toxicity, and changes in tumor HER2 and TROP2 expression. Quality-of-life change is assessed in the first-treatment cohort.

Burden on patient: Moderate to high. Treatment requires visits every three weeks, imaging approximately every nine weeks, blood tests and questionnaires, plus cardiac testing with T-DXd or eye examinations with Dato-DXd. Research biopsies are requested at baseline, early during first treatment, and at progression or crossover when a tumor is safely accessible; these add procedural burden beyond routine care. Patients who receive both drugs may undergo a prolonged series of visits and assessments, followed by approximately six-monthly long-term follow-up.
