LY4052031 is an investigational, next-generation antibody–drug conjugate (ADC) targeting Nectin-4, a cell-surface protein overexpressed in urothelial and several other cancers. It is being evaluated in the first-in-human, open-label phase 1 NEXUS-01 trial (NCT06465069), which is enrolling patients with advanced or metastatic urothelial carcinoma and selected solid tumors. No peer-reviewed full-length clinical publication has yet been reported. (ascopubs.org)
LY4052031 comprises a humanized IgG1 anti–Nectin-4 antibody linked through a cleavable peptide linker to Camp98 (LSN3889710), a camptothecin-derived topoisomerase I inhibitor. It has a homogeneous drug-to-antibody ratio of 8. After binding Nectin-4 and internalization, the payload is released, inhibiting topoisomerase I and thereby causing DNA damage, cell-cycle arrest, and tumor-cell death. (cancer.gov)
Initial phase 1 results were reported at ASCO 2026. As of November 25, 2025, 70 patients had received 0.6–5.4 mg/kg intravenously every three weeks, including 47 with metastatic urothelial carcinoma.
Among 21 efficacy-evaluable urothelial-cancer patients with adequate CYP2D6 activity treated at 2.4–4.8 mg/kg:
These are preliminary results from small, nonrandomized dose-escalation cohorts and do not establish comparative efficacy. (ascopubs.org)
The most frequent treatment-emergent adverse events were nausea (43%), alopecia (39%), fatigue (37%), decreased appetite (29%), and constipation, diarrhea, altered taste, mucositis, and vomiting (27% each). Anemia occurred in 24% and was the most common grade 3 or higher event (13%).
Reduced CYP2D6 metabolic activity was associated with greater payload exposure and more dose-limiting toxicity at higher doses. The protocol was consequently amended to require CYP2D6 genotyping and separate dose finding for patients with low activity scores. Dose optimization remains ongoing. (ascopubs.org)

Goal: Determine a safe, tolerable dose of LY4052031 and assess its antitumor activity in advanced or metastatic urothelial carcinoma and selected other solid tumors.
Patients: Participants have advanced or metastatic urothelial carcinoma or selected other solid tumors and an ECOG performance status of 0–1. Urothelial carcinoma cohorts require at least one prior regimen for advanced disease and include separate expansion cohorts for patients with and without prior enfortumab vedotin. Other tumor cohorts require exhaustion of appropriate standard therapies or no available standard therapy.
Design: Multicenter, open-label, nonrandomized dose-escalation and dose-optimization study followed by tumor- and treatment-history–defined expansion cohorts.
Treatments: All participants receive intravenous LY4052031; doses are escalated or compared before an optimal dose is selected for expansion. This investigational antibody–drug conjugate binds Nectin-4 and delivers a topoisomerase I inhibitor payload to tumor cells. In preliminary, small nonrandomized cohorts, 10 of 21 efficacy-evaluable patients with urothelial carcinoma responded, including 6 of 15 previously treated with enfortumab vedotin; these findings do not establish comparative efficacy.
Outcomes: The phase 1a primary measure is dose-limiting toxicities during the first 21-day cycle to guide dose selection. The phase 1b primary measure is investigator-assessed objective response rate by RECIST 1.1. Secondary measures include pharmacokinetics, duration and time to response, disease control, progression-free survival, and overall survival.
Burden on patient: High, particularly during dose finding: participants need repeated visits for intravenous treatment, first-cycle safety monitoring and pharmacokinetic blood draws, as well as ongoing imaging and assessments. Archival tumor tissue is required, or a screening biopsy if permitted and tissue is unavailable. Visits and follow-up may extend over a prolonged period.
