Goal: Determine whether adding PRGN-2009 to pembrolizumab improves tumor response in pembrolizumab-resistant recurrent or metastatic cervical cancer, while evaluating safety.

Patients: Adults with PD-L1–positive, HPV16/18-positive recurrent or metastatic cervical cancer previously treated with pembrolizumab. Patients must have measurable disease, ECOG performance status 0–1, and no more than two prior systemic regimens for recurrent or metastatic disease.

Design: Multicenter, open-label trial with 1:1 randomization to combination treatment or pembrolizumab alone.

Treatments: The experimental arm receives PRGN-2009 at 5 × 10^11 particle units every 3 weeks for three doses, then every 6 weeks, plus pembrolizumab 400 mg every 6 weeks. PRGN-2009 is an investigational gorilla adenoviral-vector vaccine designed to induce T-cell responses against HPV16/18 E6 and E7 and HPV16 E5 proteins. In a small, nonrandomized phase 1 study, no objective responses occurred among six patients receiving PRGN-2009 alone; two of 10 response-evaluable patients responded when it was combined with bintrafusp alfa, but PRGN-2009’s contribution is uncertain. The comparator arm receives pembrolizumab 400 mg every 6 weeks, a PD-1 inhibitor given alone in this study.

Outcomes: The primary endpoint is objective response rate by RECIST v1.1 at 1 year. Secondary measures include treatment-emergent adverse events, progression-free and overall survival, best overall response, disease control rate, time to response, duration of response, and adenoviral-vector shedding through 4 months.

Burden on patient: Moderate. Patients require repeated treatment visits, including visits every 3 weeks during the first three PRGN-2009 administrations in the combination arm, followed by every-6-week dosing; the comparator arm receives treatment every 6 weeks. Response imaging and safety monitoring are required, and the combination arm also has samples collected to assess vector shedding. The available information does not specify mandatory biopsies or intensive pharmacokinetic sampling.
