Goal: Assess the safety, tolerability, pharmacokinetics, immunogenicity, and preliminary antitumor activity of CTX-8371, and select doses for further study.

Patients: Adults with measurable, locally advanced unresectable or metastatic melanoma, head and neck squamous cell carcinoma, non-small-cell lung cancer, triple-negative breast cancer, or classical Hodgkin lymphoma after specified prior therapies. Most solid-tumor groups must have progressed after PD-1/PD-L1 treatment. Patients need an ECOG performance status of 0–1 and adequate organ function; significant prior checkpoint-inhibitor toxicity and active autoimmune disease are key exclusions.

Design: First-in-human, open-label, nonrandomized study with a 3+3 dose-escalation cohort followed by dose expansion. There is no control arm.

Treatments: CTX-8371 is given as intravenous monotherapy every 2 weeks. Dose escalation evaluates five levels from 0.1 to 10 mg/kg; expansion doses are selected using escalation data. CTX-8371 is a bispecific antibody targeting PD-1 and PD-L1; preclinical work suggests it may also promote removal of PD-1 from activated T-cell surfaces. Early dose-escalation results showed responses in 3 of 15 evaluable patients, with no reported dose-limiting toxicities; these small, uncontrolled data do not establish efficacy.

Outcomes: Primary measures are dose-limiting toxicities, treatment-emergent adverse events, laboratory abnormalities, and selection of doses for further study. Secondary measures include objective response, duration of response, progression-free and overall survival, pharmacokinetic parameters, dose response, and anti-drug antibodies. Tumor response is assessed using RECIST 1.1 or Lugano criteria, as appropriate.

Burden on patient: High: intravenous infusions every 2 weeks require repeated site visits, with blood sampling for safety, pharmacokinetics, and immunogenicity plus imaging for response assessment. Safety follow-up continues for 30 days after the last dose, and efficacy follow-up may extend to 2 years. No mandatory research biopsy is specified.
