A2B694 is an investigational, patient-derived CAR T-cell therapy being studied in the open-label phase 1/2 EVEREST-2 trial for advanced solid tumors that express mesothelin and have lost HLA-A*02. Human results are available, but they are preliminary and come from a small, nonrandomized group. (ascopubs.org)
A2B694 combines a mesothelin-targeting CAR that activates the T cell with an HLA-A02–recognizing inhibitory receptor. The design aims to attack mesothelin-positive tumor cells that have lost HLA-A02 while sparing cells that retain it. This selectivity remains a proposed mechanism, not a demonstrated clinical benefit. (cancer.gov)
In the 2026 conference abstract, 13 patients had received A2B694 and were evaluable as of January 5, 2026. One patient with non-small-cell lung cancer had a confirmed complete response—1 of 13 patients overall. That patient subsequently had a central nervous system relapse on day 243, while the response outside the central nervous system persisted at day 284. The abstract does not establish an overall response rate across tumor types or show how A2B694 compares with other treatments. (openalex.org)
The same abstract reported grade 3 neutropenia in two patients, grade 3 immune effector cell–associated neurotoxicity syndrome (ICANS) in one, and grade 1 cytokine release syndrome in one. No dose-limiting toxicities were reported with up to 17 months of follow-up; dose escalation was continuing. The small sample limits conclusions about less common or longer-term harms. (ascopubs.org)

Goal: Determine a safe recommended dose of A2B694 and assess its antitumor activity and safety in selected advanced solid tumors.
Patients: Adults with previously treated, recurrent unresectable, locally advanced, or metastatic mesothelin-positive solid tumors, including colorectal, pancreatic, non-small cell lung, ovarian cancer, and mesothelioma. Patients must be germline HLA-A*02 heterozygous, have tumor loss of HLA-A*02, measurable disease, ECOG performance status 0–1, and stored cells collected through BASECAMP-1.
Design: Multicenter, open-label, nonrandomized, single-arm seamless phase 1/2 study. Phase 1 uses dose escalation to select a recommended phase 2 dose; phase 2 evaluates response at that dose.
Treatments: After BASECAMP-1 apheresis and preconditioning lymphodepletion, patients receive one intravenous infusion of autologous A2B694 Tmod CAR T cells. A2B694 uses a mesothelin-targeting activating receptor and an HLA-A*02-recognizing inhibitory receptor, designed to attack mesothelin-positive tumor cells that have lost HLA-A*02 while sparing cells that retain it; clinical selectivity remains unproven. Preliminary data from 13 evaluable recipients reported one confirmed complete response in NSCLC, followed by CNS relapse, and no dose-limiting toxicities; the small nonrandomized sample does not establish efficacy or comparative safety.
Outcomes: Phase 1 measures adverse events and dose-limiting toxicities by dose level and determines the recommended phase 2 dose. Phase 2 measures independently reviewed objective response rate by RECIST v1.1. Secondary measures include circulating CAR T-cell persistence and cytokine levels, with follow-up extending to 24 months after infusion.
Burden on patient: High. Participation requires separate BASECAMP-1 enrollment and apheresis, tumor testing, lymphodepleting treatment, an infusion visit, and repeated safety assessments, blood sampling, and response imaging over up to two years. CAR T-cell toxicity monitoring and travel to participating centers may add substantial demands; the exact visit frequency is not specified.

Caution: ClinicalTrials.gov appears to have newer eligibility criteria than the version saved here. Review the current criteria in ClinicalTrials.gov.
Inclusion Criteria:
Key Inclusion Criteria:
