Investigational Drug
Tiragolumab (MTIG7192A; RG6058; RO7092284) is an investigational, fully human IgG1κ monoclonal antibody developed to treat cancer. It has primarily been evaluated intravenously with the anti-PD-L1 antibody atezolizumab, sometimes alongside chemotherapy. Clinical activity has varied substantially by tumor type: phase III benefit was demonstrated in esophageal squamous-cell carcinoma, whereas phase III trials in non-small-cell and small-cell lung cancers did not meet their primary efficacy endpoints. (pubmed.ncbi.nlm.nih.gov)
Tiragolumab targets TIGIT, an inhibitory immune-checkpoint receptor expressed on activated T cells, natural-killer cells and regulatory T cells. It blocks TIGIT binding to CD155/PVR, potentially restoring antitumor immune activity and complementing PD-L1 blockade. Tiragolumab has an active Fc region; translational and preclinical evidence suggests that Fc-receptor engagement can activate tumor-associated myeloid cells, reduce suppressive regulatory-T-cell programs and promote a less exhausted, more memory-like CD8-positive T-cell state. (pubmed.ncbi.nlm.nih.gov)
Tiragolumab-containing regimens have generally shown safety profiles consistent with their individual components, although severe and fatal events have occurred.

TrialFetch AI summary: Adults with previously treated metastatic colorectal adenocarcinoma, ECOG 0–1, and an eligible biomarker are assigned to biomarker-directed combination arms. Regimens pair the PI3Kα inhibitor inavolisib, anti-TIGIT antibody tiragolumab, CDK7 inhibitor SY-5609, or KRAS G12C inhibitor divarasib with cetuximab, bevacizumab, atezolizumab, and/or FOLFOX or FOLFIRI, depending on the arm.
ClinicalTrials.gov ID: NCT04929223
TrialFetch AI summary: Patients with measurable recurrent or persistent endometrial carcinoma after 1–2 prior systemic therapies are assigned to biomarker-matched cohorts. Open cohorts evaluate atezolizumab (anti–PD-L1) plus talazoparib (PARP inhibitor) for high genomic loss of heterozygosity or tiragolumab (anti-TIGIT) for MSI-high/high-TMB tumors, and inavolisib (PI3Kα inhibitor) plus letrozole for PIK3CA-mutant tumors or giredestrant (ER degrader) plus abemaciclib (CDK4/6 inhibitor) for RB1-intact, low-grade ER-positive tumors.
ClinicalTrials.gov ID: NCT04486352
TrialFetch AI summary: Patients with previously untreated, unresectable locally advanced or metastatic HCC, ECOG 0–1, and Child–Pugh A liver function are randomized to atezolizumab (PD-L1 inhibitor) plus bevacizumab (VEGF inhibitor) or an investigational combination. Experimental arms add tiragolumab (TIGIT), tocilizumab (IL-6 receptor), amezalpat (PPARα), muzastotug (CTLA-4), IO-108 (LILRB2/ILT4), or imdatifan (HIF-2α) to that backbone, or combine bevacizumab with tobemstomig (PD-1/LAG-3).
